You will notice some changes the next time you visit Hawks Prairie Vision Clinic. We are taking precautions to secure the comfort of our customers and employees. Please prepare for the following changes:
- It is mandatory that you wear a face mask inside this office. Unfortunately, we cannot supply face masks to customers and patients.
- Your temperature will be taken at the door. If your temperature exceeds 100 degrees, we will kindly ask that you leave the office and call us to reschedule your visit.
- If you display any cold symptoms, you may be asked to reschedule or return at a later date.
- Maintain social distancing by 6 feet. We have limited and spaced seating in our lobby.
- We ask that you come alone for your visits. If you do come with someone, we ask that they wait in the car. Please make appropriate arrangements for your children to stay home during your appointment time. If your child is the patient, only one parent or guardian is permitted to accompany during the visit. If a patient needs a family member or caregiver to assist, please bring that up during the time of scheduling and we would be more than happy to accommodate.
- We frequently sanitize all surfaces, equipment, handles, pens, clipboards, armchairs, etc. We wash and sanitize our hands frequently as well.
- When we have a maximum of 5 customers in the front office, any additional visitors may be asked to wait in their car.
