Office Policies
**Beginning January 1, 2019, our office will be charging a "no show" fee of $25. This fee cannot be covered by any insurance company, it will be the patient's responsibility to pay this fee in order to be seen for their next exam.
A "no show" means, if you have a scheduled appointment with our office and do not show up or call, you have "no showed" to your appointment. If you call and reschedule or cancel an appointment prior to your appointment time, you will not be charged this fee.
**Beginning January 1, 2019, our office will no longer bill a routine eye exam copay to any secondary vision insurance. If your primary vision policy has a copay for a routine vision check, please be prepared to pay that copay at the time of service.
However, we will bill secondary medical insurances for medically related office visits. We expect all patients to give us all of their insurance information so we can bill services appropriately.
For any medically related visits to our office, we bill your primary medical insurance (and/or secondary medical insurance) in full for the services. We do not collect a copay, coinsurance, or deductible up front at the time of the exam. Your insurance company will calculate what your copay, coinsurance, and deductible out of pocket costs are and they send us an Explanation of Payment (EOP). With the EOP information, we will send you a Statement of Charges bill in the mail to collect any out of pocket costs.
If you do not document on our office forms or verbally update our staff with insurance changes, we may not be able to re-bill denied services, which results in patient financial responsibility.
Medically related services are:
**For any members with Coordinated Care, Ambetter, or Envolve vision coverage, we are contracted to provide services for your annual routine eye exam only. We are not contracted to provide contact lens exams, order contacts, or glasses under your vision plan.
**Please be advised that ALL prescription glasses are non-returnable and non-refundable**
Our Optical Team will do their very best to assist in creating the perfect set of eyeglasses for you. However, if there is ever an issue with the prescription or the fit of the frame that needs to be resolved, our Team can restyle the frame or lens options to better suite your needs and comfort.

- Vision Fields Test (peripheral vision test)
- OCT Test (an imaging test of your retina)
- Fundus Photo (an imaging picture of your retina, optic disc, and macula)
- Eye Infections
- Growths on Eyelids
- Flashes and Floaters
- Pre and Post operative visits in relation to eye surgeries
- Punctal Plugs
- Office Visits regarding anything other than your annual routine eye exam
